NOTICE: Residents of Connecticut, Louisiana, Maine, Maryland, North Carolina, Utah and Washington State: Residents of the States of Connecticut, Louisiana, Maine, Maryland, North Carolina, Utah and Washington State who wish to sign up as an Independent Business Owner with AmeriPlan are required to pay a one-time fee of $45 which includes a $20 registration and a $25 IBO Training Kit. Residents of these States that sign up as an IBO also have the option to purchase the $50 per month IBO Benefits Program which includes all IBO Benefits and, at no extra cost, a household membership in the AmeriPlan Dental Plus Program and Four (4) e-commerce enabled websites. If these residents who sign up as an IBO do not wish to purchase the optional IBO Benefits Program, they will not receive the IBO Benefits, household membership in the AmeriPlan Dental Plus Program and Four (4) e-commerce enabled websites.
